THE FOLLOWING INFORMATION IS DESIGNED TO HELP PARENTS AND SCHOOL AGE CHILDREN WITH GUIDANCE FROM  SCHOOL AGE YEARS THROUGH THE GRADUATION PROCESS. MORE INFORMATION IS AVAILABLE BY CONTACTING THE OFFICE OF LAURA K. MARTIN, ADULT SERVICES COORDINATOR.

TIMELINE FOR PARENTS AND HIGH SCHOOL-AGE STUDENTS

AGE 16

Contact Ross County Board of Developmental Disabilities at 740.775.7044 to make sure that your child is eligible for county board services. You must reapply for Services when a child turns 16 even if they were found eligible in the past. (If found eligible in the past, you will receive a letter from the Intake Department).

JUNIOR YEAR

Take advantage of any vocational programs that are available through your High School. Consider applying for BVR (Bureau of Vocational Rehabilitation) services to help with your child’s vocational goals.

Invite your Ross DD Service and Support Administrator (SSA) to your child’s IEP meetings.

AGE 18

Apply for SSI (Social Security Income). Social Security can take up to six months to make a decision. Call locally at 775.0206 to make an appointment. You will need to gather documentation of your child’s disability (IEP, MFE, assessments from doctors or psychologists, etc.) for the appointment. A Ross DD Service and Support Administrator can help guide you through the process and attend the appointment by request.

Apply for Medicaid Health Insurance. It is easier to apply for Medicaid after your child has been approved for SSI. A Ross DD Service and Support Administrator can help guide you through the process at your request.

Apply for legal guardianship through the probate court (if needed). An attorney is NOT needed to apply for guardianship. A Ross DD Service and Support Administrator can help guide you through the process.

Call your Ross DD Service and Support Administrator if you need any help.

Invite your Ross DD Service and Support Administrator to your child’s IEP meetings.

Graduation Year

Apply for SSI (if you haven’t already done so). Social Security can take up to six months to make a decision. A Ross DD Service and Support Administrator can help guide you through the process.

Apply for legal guardianship through the probate court, if you haven’t already done so and if this is needed. You will not need to complete this process if your child is capable of making informed decisions in important life areas (healthcare decisions, financial decisions, etc).

Apply for Medicaid (if you haven’t already done so). A Ross DD Service and Support Administrator can help guide you through the process.

Make sure that you have your child’s Birth Certificate and Social Security Card. The Ross County Board of Developmental Disabilities will need a copy of the Birth Certificate, Social Security Card, and a copy of the current Medicaid Card or Health Insurance card.

Consider getting a State ID at the Bureau of Motor Vehicles. A State ID is sometimes needed for employment. Contact your Ross DD Service and Support Administrator if you have questions.

Invite your Ross DD Service and Support Administrator to your child’s IEP meetings!

Work with your Ross DD Service and Support Administrator to ensure a smooth transition to Adult Services.

Not for certain if you currently have a Ross DD Service and Support Administrator (SSA)? or if you know you do not have a SSA assigned to your child and would like one to help with the transition process, please call 740.775.7044 and ask to speak with our Intake Coordinator.

RESIDENTIAL SERVICES
Residential services from Pioneer Center are based on the philosophy that all people with mental retardation and/or developmental disabilities should live as independently as they are able. For some, that may mean living in an apartment or house of their own and receiving only minimal assistance from our agency. Others may require 24-hour supervision and/or medical care. Our focus is to provide for each person the most independent and personally satisfying home environment. Individuals are encouraged to be involved in their choice of where and with whom they live and who will provide their support services.

Currently the Pioneer Center provides residential services through contracts with state certified providers. Funding for services can be made through Supported Living or Medicaid waiver programs (Individual Options Waiver or Level 1 Waiver). Our community group homes serve 43 residents in throughout the community and more than 40 individuals live in homes of their own choosing through supported living and community waivers. People interested in residential services should contact their Service & Support Administrator (SSA) concerning placement upon the appropriate waiting list or the Residential Service Director for more information.

Frontier Development Corporation Homes

The Frontier Development Corporation purchases and renovates homes that are available to consumers as another residential choice. Homes are rented to groups of no more than four consumers who share living expenses and household chores. The Housing Corporation uses rental revenues for repairs and additional housing purchases. The Residential Service Director and the Service & Support Administration staff do placement and monitoring of services. The Frontier Development Corporation currently owns 14 homes in Ross County, including one emergency respite location. 26 people live in these homes.
